Failed to import data - import failure, Index out of range

Built an IDEA app.  First, it was missing columns, then delete, redo.  matched all columns and now, import failure happens.  Tried deleting app, and redoing again.  Same results.

thoughts are appreciated in advance.

Answer:  When importing, ensure EXCEL is formatted properly.
