Task without name and App failing and visible in Operations Monitor

I am checking my Operations Monitor and getting a task named 'Not Available' without any app. As per the monitor it has been failing and taking long execution time.

But I am unable to find such task or app in the QMC. 

How can I search this 'task' and 'app' and what should I do to fix this?
