Dimension Columns in a pivot table: How to get totals and sort in a specific manner

In a pivot table report, I've set the book title in rows and used Author (dimension) as columns.

1. For each author column, I would like a total. 

2. To sort, I would like the column orders to be

- Hal Elrod
- ILPUS

then for the rest of the authors, I'd like them to be sorted alphabetically

How would I achieve this?
