How do I migrate my Qlik Sense Enterprise server to new machine with separate drives for programs and data?

My existing server has just a C drive and a single node.  The new server set up by my IT department has a C drive for the program files and a D drive for the data files.  If I back up my old server and restore to the new server, how can I set it up so that the QlikShare and QlikData folders are on the D drive and the QMC recognize where all of the apps are?

That is no problem at all. The data files are accessed through one or more file shares.
That file share can be located on any of your local drives or on a separate file server.
In QMC you configure this under CONFIGURE SYSTEM/Service cluster/CLUSTER SETTINGS/ where you have the folder paths.
You can move this any time you want by moving the physical folders,
setting up a new file share to the new location and change the configuration.
If you want to perform the move and reconfiguration with Qlik Sense down (safest) you can access this configuration from the QlikSenseUtil.exe tools which is found in 
C:\Program Files\Qlik\Sense\Repository\Util\qlik-sense-util\
It connects to the repository database and you can change some configuration in this tool without having to use the QMC. The "Service cluster" tab is where you want to look after you have connected to the repository.
Note that the repository database needs to be running in order to use QlikSenseUtil.exe but all other Qlik Sense services can be shut down in order to make sure nothing is accessing the service cluster shared folders while doing the move.

Ok...I installed Qlik Sense Enterprise on the new server and when I was prompted for the location of the share and data folders I used the UNC path for the shared folders I created on the D drive.  I have not yet done a restore of the backup from the old server to the new server.  When I do the restore of the backup to the new server, will it recognize how the new server's installation was done?  Or do I do the restore with services turned off and then use the utility to change the folder locations because the restore won't know how I set it up?

It will not pick up your new folder location, the folder location is stored in repository database which you are going to restore from your old server. Restoring the repository database the database service needs to be running but everything else should be turned off. After that you can use the tool to correct the folder location. You also need to consider different server name and possibly a new domain if that is applicable.
